Summary

We are seeking a compassionate and dedicated Social Worker to join our team in Huntsville, AL. In this role, you will play a vital part in supporting our patients' mental health and well-being through counseling and case management services. Your contributions will help us fulfill our mission of enhancing the quality of life for those we serve.
Duties and Responsibilities

1. Complete Hormone Therapy (HT) intake process, including but not limited to requesting and obtaining necessary Medical Records from other providers, making appointments with the medical provider, and following up on the appointment completion.

2. Conduct psychosocial assessments, develops treatment plans, and provides intervention and treatment as indicated related to Hormone Therapy medical treatment.

3. Assess individuals for the Hormone Therapy readiness assessment.

4. Provides educational services to patients, staff, and providers.

5. Assist with processing internal and external referrals, including working with insurance carriers to secure coverage for care, and ensuring that outgoing referral documentation is appropriate and complete.

6. Fields all calls from the community regarding individual issues and provides information, psychosocial support, education, and resources to all callers regardless of client status.

7. Maintain an active list of all patients accessing hormone care, including any preparation for or awaiting surgery, and conduct periodic reassessments of their psychosocial status.

8. Screen for unmet psychosocial needs and help coordinate referrals and resources to help meet these needs.

9. Educate patients on community resources.

10. Provide social work consultation to other disciplines within the scope of this job description and the community.

11. Assist, make appointment, and follow up on appointment completion for clients by referring them for additional supportive services, including housing, dental, mental health, eyeglasses, and behavioral healthcare, including substance abuse services.

12. Assists with internal operations and outreach activities.

13. In collaboration with Director of Patient Access, provides ongoing support, education, and consultation to Patient Services staff and supervisors to build and maintain competency and sensitivity in services and interactions.

14. Assist clients with questions regarding prescription drug coverage, including applications to pharmaceutical patient assistance programs and medical insurance through the Affordable Health Care Act.

15. Assist with the completion of daily chart reviews for Ryan White clients listed on schedule to ensure adherence and eligibility.

16. Complete daily documentation in the electronic health record (eClinicWorks).

17. Submit monthly documentation and reports, including billing sheets, to the Director of Case Management Services.

18. Develop and maintain strong working relationships with and routinely consult with case managers, healthcare providers, behavioral healthcare providers, psychiatrists, and health educators both within the agency and the community at large; participate in huddles and care team meetings as requested and able.

19. Attend bi-monthly staff and department meetings.

20. Provide clients with transportation as needed to medical and behavioral healthcare appointments.

21. Maintains strict confidentiality of employee, client, company, and vendor data, as well as handling sensitive and critical information with professionalism and discretion.

22. All other duties, as assigned.

Knowledge, Skills, and Abilities

1. Knowledge of the concepts and principles of social work, the National Association of Social Workers Code of Ethics and Alabama Board of Social Work Licensure Statutes.

2. Ability to establish and maintain effective team relationships with agency coworkers, other community organizations and the public.

3. Ability to prioritize needs and plan work assignments accordingly, including effective time management skills.

4. Fluency in basic computer systems, including Microsoft Office.

5. Knowledge of HIPAA confidentiality requirements.

Minimum Qualifications

Master’s degree or higher in social work with licensure by the State of Alabama (LMSW).

At least two (2) years of experience working with the affected community.

Bilingual in both English and Spanish language preferred but not required.
