Summary
The Licensed Clinical Social Worker is a key member of the Rush University System for Health community. This position provides social work services from a variety of referral sources and connects those patients and community members to relevant resources both internal to rush and in the community. This position supports relevant programming for the aforementioned population also and may serve as a representative of their department in meetings and presentations. This position exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.

Other Information
Required Job Qualifications:
- Master’s Degree in Social Work.
- Licensed Social Worker (LCSW) in State of Illinois.
- Two years’ experience in healthcare, care management, mental health and/or social services.
- Experience with relevant population(s).
- Professional qualities such as initiative, self-reflection, commitment, responsiveness and adaptability.
- Knowledge of care management, community-based interventions, assessment, counseling and/or motivational interviewing.
- Computer literate (Microsoft Office and others).

Responsibilities
- Job Responsibilities:
- Works directly with patients and their families to address unmet needs and provide the necessary social work services them achieve their
- Carries out comprehensive social work assessments by phone or in-person to develop a plan of care for the patient and/or family.
- Connects patients and their families to relevant resources, both at Rush and in the community, to address their needs.
- Provides care management and/or support programming for aforementioned population as outlined by Manager.
- Documents all client interactions in either the Epic, LCM software and/or other spaces as indicated by Manager.
- Maintains relationships with referring Rush clinics through consistent and timely communication.
- Promotes Rush University System for Health clinical services by referring clients to relevant Rush clinics.
- Represents the department at meetings, conferences or other public engagements on an as-needed basis.
