Qualifications:

• Must be a licensed Social Worker in the state of Ohio
• Hospice or Long-term care experience preferred

Job Duties:

• Conduct psychosocial assessment and develop individualized plan of care for patient and family as determined by assessment.
• Provide individual and family grief counseling.
• Complete regular visits to patient and family members as outlined in patient plan of care.
• Maintain complete and timely documentation that clearly reflects interventions as outlined in patient’s plan of care.
• Provide connection to community supports and services as needed.
• Complete all necessary paperwork in timely fashion, in accordance with Medicare guidelines.
• Provide on-call coverage on regular basis.
• Communicate regularly with families and ECF staff regarding patient process and/or changes in condition.
• Provide bereavement counseling to families after a death, as needed.
• All other duties as assigned by the Director
