Qualifications:
- Must be a licensed Social Worker in the state of Ohio
- Hospice or Long-term care experience preferred
Job Duties:
- Conduct psychosocial assessment and develop individualized plan of care for patient and family as determined by assessment.
- Provide individual and family grief counseling.
- Complete regular visits to patient and family members as outlined in patient plan of care.
- Maintain complete and timely documentation that clearly reflects interventions as outlined in patient’s plan of care.
- Provide connection to community supports and services as needed.
- Complete all necessary paperwork in timely fashion, in accordance with Medicare guidelines.
- Provide on-call coverage on regular basis.
- Communicate regularly with families and ECF staff regarding patient process and/or changes in condition.
- Provide bereavement counseling to families after a death, as needed.
- All other duties as assigned by the Director
