Position Overview

The Resident Care Coordinator plays a critical role in leading and supporting the caregiving team to deliver exceptional care and services to residents. This position is responsible for overseeing daily operations, ensuring compliance with health and safety standards, and supporting both residents and care team members. As a Resident Care Coordinator, you will work closely with the Executive Director and leadership team to uphold company standards of resident care.

Key Responsibilities

Resident Care Oversight:

  • Ensure that residents receive daily care as per their individual care plans, including personal care, medication management, and support with daily living activities.
  • Monitor resident well-being, documenting and addressing any changes in condition or care needs.
  • Coordinate and facilitate resident activities, outings, and monthly events, ensuring a high quality of life for all residents.

Team Leadership and Support:

  • Lead oversight and development of the team and weekly meetings to provide updates, set expectations, and foster a supportive team environment.
  • Mentor and oversee the training of new team members, ensuring they understand quality standards and core values.
  • Provide ongoing guidance and support to care team members, address questions, and overseeing performance to maintain company values and standards.

Compliance and Safety:

  • Conduct monthly compliance audits for medication administration, emergency protocols, and safety checks (e.g., fire drills, disaster preparedness).
  • Ensure that all resident care documentation is accurate and up-to-date, including medication records, ADLs, and incident reports.
  • Maintain household supplies and oversee inventory, reporting needs to the Executive Director.

Communication and Coordination:

  • Serve as the primary contact for residents’ families, providing updates and addressing any concerns regarding care.
  • Liaise with the Executive Director regularly to discuss team performance, resident care updates, and any operational challenges.
  • Assist with coordinating resident move-ins and transitions, ensuring a welcoming and smooth process.

Qualifications:

  • High school diploma or equivalent required; further education in healthcare or caregiving is a plus.
  • At least five years of experience in a caregiving and/or supervisory role within a senior care or similar setting.
  • Strong interpersonal and communication skills with a passion for elder care.
  • Ability to lead by example, with experience mentoring and training team members.
  • Must pass a background check and meet health and safety regulatory requirements, including TB screening and CPR certification.
  • Memory care experience is preferred.
  • Hospice care experience is preferred.

Job Types: Full-time, Part-time

Pay: $65,000.00 - $80,000.00 per year
