Vital Health is transforming behavioral healthcare in Northwest Ohio. We provide integrated treatment for primary care, mental health, and substance use disorders for children and adults. We are currently seeking a Licensed Social Worker (LSW) with experience in mental health and substance use disorder (SUD) treatment to join our team of compassionate professionals. Multiple positions available, youth and adult.

Requirements:

  • Bachelor's Degree in Social Work or related field (Master’s preferred)
  • Active licensure: LSW, LISW, LPC, or LPCC
  • Knowledge of co-occurring disorders, addiction recovery, and withdrawal management
  • Understanding of laws and regulations related to confidentiality, patient rights, and ethics
  • Strong documentation, communication, and crisis management skills
  • Ability to work flexibly and collaboratively as part of a multidisciplinary team
  • Passion for serving individuals with complex behavioral health needs

Responsibilities:

  • Conduct diagnostic assessments for new mental health/SUD patient admissions
  • Develop and update individualized treatment plans for co-occurring disorders
  • Facilitate group therapy sessions (PHP, IOP, Day Treatment levels of care)
  • Provide individual counseling for mental health and substance use disorders
  • Assist patients in accessing community support resources (housing, food, benefits)
  • Coordinate care with case managers and support staff
  • Maintain thorough, compliant documentation in the EHR system
  • Educate patients on program expectations, confidentiality rights, and treatment goals
  • Respond to patient crises and conduct home visits as needed
  • Promote a respectful, safe, and ethical care environment
  • Remind patients of program rules and regulations as necessary and report infractions appropriately.
  • De-escalate patient concerns and other patient problems.
  • Protect the privacy of all patient information in accordance with the organization's privacy policies, procedures, and practices, as required by federal and state law, and in accordance with general principles of professionalism as a health care provider.

Job Types: Full-time, Part-time

Pay: $23.00 - $29.00 per hour
