Job Summary
The Care Coordinator plays a vital role in ensuring that individuals receive the highest quality of care tailored to their unique needs. This position involves coordinating care plans, assisting with activities of daily living (ADLs), and providing support to clients in various settings, including assisted living facilities, nursing homes, and home care environments. The ideal candidate will have experience working with individuals with developmental disabilities, dementia, and Alzheimer's care, demonstrating compassion and commitment to enhancing the quality of life for those they serve.

Duties

  • Develop and implement individualized care plans in collaboration with clients, families, and healthcare professionals.
  • Assist clients with ADLs such as bathing, dressing, grooming, and meal preparation.
  • Provide direct support to individuals with disabilities, including those with autism or other developmental challenges.
  • Monitor and document vital signs and changes in clients' health status using EMR systems.
  • Administer medications as prescribed and ensure compliance with care plans.
  • Engage in conflict management and behavior management strategies to promote a positive environment.
  • Facilitate communication between clients, families, and healthcare providers to ensure continuity of care.
  • Maintain compliance with HIPAA regulations and uphold confidentiality standards.
  • Participate in training related to first aid, caregiving techniques, and medical terminology as necessary.
  • Perform light cleaning duties to maintain a safe and hygienic environment for clients.

Qualifications

  • Previous experience in senior care settings such as assisted living or nursing homes is preferred.
  • Familiarity with memory care practices and Alzheimer’s care techniques is highly desirable.
  • Strong understanding of patient care protocols, including vital sign monitoring and medication administration.
  • Experience working directly with individuals with disabilities or special needs is essential.
  • Ability to lift heavy objects safely (e.g., using a Hoyer lift) when assisting clients.
  • Excellent communication skills for effective interaction with clients, families, and team members.
  • Knowledge of case management principles and the ability to create effective care plans.
  • A background in social work or nursing is beneficial but not required.
  • Valid driver's license may be required for transportation purposes related to client needs. This role is ideal for compassionate individuals dedicated to making a difference in the lives of others through quality care coordination and support.

Job Type: Full-time

Pay: $23.36 - $28.13 per hour
