Clinical Social Worker
The Clinical Social Worker provides direct services to adult and geriatric veterans enrolled in the Home Based Primary Care (HBPC) Program. This role involves high-quality case management, clinical treatment, advocacy, and coordination with VA and community service providers. The Clinical Social Worker collaborates with the interdisciplinary HBPC Team or other treatment teams.
Major duties include:
- Screening/Assessment: Develop assessments of the veteran in collaboration with the interdisciplinary team.
- Treatment Planning/Goal Setting: Contribute to the development of the treatment plan and set achievable goals.
- Referral to Service Providers: Serve as the expert on VA and community resources.
- Resource Development: Enhance the content of the social work resource file.
- Advocacy: Act as an advocate for the veterans best interests.
- Crisis Intervention: Develop crisis management plans.
- Education/Health Promotion and Prevention: Provide education related to VA and community resources.
- Coordination of Services: Coordinate non-institutional and institutional services.
- Supportive Counseling: Provide ongoing supportive counseling to veterans and their caregivers.
- Administrative Responsibilities: Enter all veteran/family contacts in the electronic record.
Work Schedule: Monday-Friday, 07:30am-4:00pm.
Pay: Competitive salary with regular salary increases.

Requirements:
- U.S. Citizenship required.
- Selective Service Registration required for males born after 12/31/1959.
- Proficiency in written and spoken English.
- Must pass pre-employment physical examination.
- Participation in the seasonal influenza vaccination program is required.
Qualifications:
Basic Requirements:
- United States Citizenship.
- Education: Masters degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).
- Licensure: Licensed or certified by a state to independently practice social work at the masters degree level.
Grade Determinations:
- GS-11: One year of post-MSW experience or a doctoral degree in social work.
Physical Requirements: Sedentary work with occasional standing, walking, bending, twisting, and carrying light items.
