Crossroads is an Arizona Department of Health Services licensed substance abuse treatment provider with proven expertise in serving men, women, and Veterans with Substance Use Disorder through the provision of high quality, successful, and affordable residential and outpatient recovery programs.

Job Summary: The Clinical Care Coordinator is responsible for providing coordination to their CCOM teams which provide case management services to adults with chronic mental illness, substance abuse, and physical health conditions. The goal of care coordination is to reduce unnecessary contacts with emergency departments and unnecessary inpatient hospitalizations while removing barriers to outpatient services and healthy outcomes.

Supervisory Responsibilities:

  • Recruits, interviews, hires, and trains employees.
  • Provides constructive and timely performance evaluations.
  • Recommends compensation adjustments.
  • Handles discipline and termination of employees in accordance with company policy.
  • Involved with handling all employee relation issues.
  • Timecard responsibilities.
  • PTO management.

Essential Functions:

  • Designates appropriate referrals, coordination, and follow-up to the Community Care Specialists (CCS) for patients to receive services and support.
  • Oversees CCS to provide health care appointment scheduling for patients with mental health, substance abuse, and physical health providers.
  • Monitoring monthly billable goal of each CCS and ensuring measures are met.
  • Maintaining the CCOM census by scheduling intakes as well as by completing discharges when needed.
  • Assigning outreach and client contact to each staff member to avoid gaps in care over 30 days.
  • Works closely with community resources and organizations to build strong partnerships.
  • Complete and file all documentation according to Crossroads Policy and Procedures.
  • Ensure releases of information and other required documents are completed and filed.
  • Attend provider team meetings to promote collaboration of service and treatment providers.
  • Attend and participate in individual and/or peer supervision as directed.
  • Provide monthly meeting supervision with CCS staff.
  • Performs other duties, as needed.

Qualifications:

  • Current CPR/First Aid/Narcan training.
  • Current and annual negative TB Test.
  • Current and valid Fingerprint Clearance Card.
  • Clean Motor Vehicle Record (MVR) check performed annually and upon hire, preferred.
  • Successfully pass a background check and an Arizona Department of Health Services Adult Protective Services check.
  • Management experience preferred.

Knowledge, Skills, and Abilities:

  • Provide strong leadership qualities as well as problem solving skills.
  • Demonstrated strong organizational skills.
  • Ability to communicate effectively at multiple levels, both verbal and written.
  • Ability to work independently and as a member of a team.

Education & Experience:

  • Minimum of High School diploma or GED required.
  • 2-3 years behavioral health experience preferred.
  • Computer/Microsoft office experience including EMR experience.

Work Environment:

  • Fast paced behavioral health setting.
  • Primarily indoors, however, some outdoor
    work may be requested.

Physical Demands:

  • While performing job duties, the employee is regularly required to sit and speak or hear. The employee is occasionally required to stand, walk, use hands to handle or feel, reach hands and arms, stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move up to 50 pounds and seldom lift and/or move >100 pounds. Specific vision abilities required by this job include color vision, depth perception, and ability to adjust focus.

Shift Requirements:

  • Days, nights, weekends, and holidays.

Travel:

  • Valid AZ Driver’s License.
  • May include travel between sites and other locations.
  • Local and statewide travel when requested or required.
