Job Overview
We are seeking a dedicated and organized Care Coordinator to join our healthcare team. The ideal candidate will play a crucial role in ensuring that patients receive comprehensive care by coordinating services and facilitating communication between patients, healthcare providers, and administrative staff. This position requires a strong understanding of medical terminology and the ability to manage multiple tasks efficiently in a fast-paced environment.
Duties
- Serve as the primary point of contact for patients, addressing their inquiries and concerns regarding their care.
- Coordinate appointments, referrals, and follow-up services to ensure seamless patient care.
- Maintain accurate patient records and documentation in compliance with healthcare regulations.
- Assist Intake Coordinator in the input/downloading of all pertinent patient information into the agency database system.
- Utilize medical terminology effectively to communicate with healthcare professionals and assist in administrative tasks.
- Provide support in managing office operations, including scheduling, billing inquiries, and insurance verification.
- Assist in the development of care plans tailored to individual patient needs.
Knowledge, Skill, and Experience:
- Minimum Education (or substitute experience) required: Must possess, as a minimum, a high school diploma or equivalent (GED).
- Minimum Experience required: On-the job training provided in staffing procedure. Experience in coordinating pre and post-hospital care preferred.
- Computer Skills required both clinical and clerical programs. Must be able to type a minimum of 45 words per minute and have a working knowledge on medical terminology.
- Possession of strong organizational skills.
- Excellent verbal and written communication skills. Possess exceptional interpersonal communication skills.
- Ability to work independently on assigned tasks as well as accept direction on given assignments.
Job Type: Full-time
Pay: $18.00 - $20.00 per hour
Expected hours: 40 per week
