Part-Time Social Worker 24-30 hours per week. Could be parttime or fulltime based on the scheduled hours.

Position Summary: Provides social services supporting skilled nursing and personal care residents and families that enhance the admission, adjustment, quality of life and discharge to residents and families. Area of focus include emotional, behavioral, and social dimensions of the resident's life.
