Job Overview
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will have a strong background in social work and be skilled in providing support to individuals and families facing various challenges. This role involves assessing client needs, developing treatment plans, and facilitating access to community resources. The Social Worker will work closely with patients, families, and interdisciplinary teams to promote mental health and well-being.
Responsibilities
- Conduct comprehensive assessments of clients' needs and develop individualized care plans.
- Provide child and family counseling, focusing on behavioral health and mental health counseling.
- Facilitate individual and group therapy sessions, utilizing techniques such as cognitive behavioral therapy and motivational interviewing.
- Collaborate with healthcare professionals to ensure holistic patient care, including hospice care and post-acute care.
- Maintain accurate medical documentation in compliance with HIPAA regulations.
- Engage in crisis intervention and management for clients experiencing acute distress.
- Conduct clinical research to improve service delivery and patient outcomes.
- Manage cases effectively, ensuring continuity of care through regular follow-ups.
- Provide support for patients with developmental disabilities, Alzheimer's care, PTSD care, dementia care, and other specialized needs.
- Utilize EMR systems for data collection and management of medical records.
Experience
- A degree in Social Work or a related field is required; licensure is preferred.
- Experience in clinical counseling, behavioral therapy, or addiction counseling is highly desirable.
- Familiarity with medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and EMR systems is beneficial.
- Previous experience working in inpatient settings, nursing homes, assisted living facilities, or outpatient clinics is a plus.
- Demonstrated ability to work effectively with children and families in crisis situations.
- Knowledge of special education processes and working with individuals on the autism spectrum is advantageous.
