Position Overview:

The Social Worker MSW - Team Lead acts in a leadership capacity within the Social Services department, guiding and supporting other social workers to ensure high-quality care. This role involves collaborating across teams, assessing psychosocial needs, providing therapeutic interventions, and managing complex cases related to abuse, neglect, guardianship, foster care, and mental health placements. The Lead also plays a critical role in crisis situations, utilizing de-escalation techniques and clinical leadership to ensure safety and effective intervention.

Key Responsibilities:

  • Act as a leader and mentor to other Social Workers, providing guidance, oversight, and support.
  • Communicate effectively with the healthcare team to facilitate safe, timely, and resourceful patient care.
  • Assess patients’ and families’ psychosocial risks, prior functioning, support systems, and coping mechanisms.
  • Counsel patients and families on emotional, social, and financial impacts of illness or disability.
  • Access and mobilize community and family resources to meet patient needs.
  • Conduct mental health assessments upon physician referral and deliver therapeutic interventions.
  • Serve as the point person for cases involving child abuse/neglect, adult/elderly abuse/neglect, institutional abuse, and domestic violence.
  • Liaise with protective agencies, guardianship, foster care, and mental health providers.
  • Support complex discharge planning and crisis management, including de-escalation in dangerous situations.
  • Develop and maintain strong relationships with community agencies, law enforcement, schools, and other stakeholders.


Qualifications & Requirements:

  • Experience: 5-7 years of Medical Social Work experience required.
  • Education: Master’s Degree in Social Work (MSW) required.
  • Licensure: Permanent Licensed Clinical Social Worker (LCSW) required.
  • Certifications: De-escalation certification required.
  • Skills & Knowledge:
    • Extensive knowledge of disease impact, human behavior, and legal mandates related to protective services.
    • Proven ability to perform psychosocial assessments, therapeutic counseling, and crisis intervention.
    • Strong leadership and mentorship skills.
    • Experience working with geographically and socio-economically diverse populations.
    • Proficiency in Microsoft Excel and Word.
    • Ability to handle dangerous or potentially dangerous situations with clinical expertise and leadership.
