Job Title: Nurse/Intake Coordinator

Job Summary: This dual-role position combines intake coordination with clinical nursing services specific to the Intensive Outpatient Program (IOP) and Partial Hospitalization Program (PHP). The staff member serves as the first point of contact for all new patients entering the practice, coordinating intake and admission processes across programs. In addition, the role provides nursing assessments and patient care within the PHP/IOP to ensure safety, comfort, and quality treatment. This position requires an active Ohio LPN or RN license and the ability to balance administrative intake responsibilities with direct clinical care.

Duties and Responsibilities:

Intake

  • Serves as the first point of contact for all new patients of the practice.
  • Conducts the phone intake process by gathering demographic and clinical information, verifying insurance, and assessing patient needs.
  • Contacts new referrals across all service lines to schedule new patient appointments

Nursing

  • Conducts nursing assessments upon admission to PHP/IOP
  • Participates in PHP/IOP multidisciplinary treatment team meetings and updates individualized treatment plans.
  • Coordinates physician-patient encounters to ensure:
  • PHP patients are seen at least weekly.
  • IOP patients are seen at least every 30 days.
  • Interacts with PHP/IOP patients in a therapeutic and supportive manner.

Demonstrates Competency in the Following Areas:

  • Philosophy and understanding of behavioral health treatment programs.
  • Knowledge and recognition of quality patient care, including strict confidentiality.
  • Knowledge and awareness of established program and personnel policies.
  • Awareness of state, federal, and Medicare regulations.
  • Ability to maintain cooperative working relationships with physicians, staff, patients, and families.
  • Professional representation of the organization in all interactions.
  • Compliance with ethical business practices and organizational policies.
  • Professional conduct in clerical, clinical, and therapeutic services.

  • Ohio RN or LPN license

  • Must possess good oral and written communication skills in the English language.
