Job Overview
We are seeking a compassionate and dedicated Medical Social Worker to join our healthcare team. The ideal candidate will be responsible for providing support and assistance to patients and their families, helping them navigate the complexities of medical care and social services. This role requires a strong understanding of clinical practices, patient assessment, and the ability to work collaboratively with healthcare professionals to ensure comprehensive patient care.

Duties

• Conduct patient assessments to evaluate needs and develop appropriate care plans.
• Provide individual and group counseling to patients dealing with mental health issues, including PTSD, addiction, and behavioral health concerns.
• Facilitate crisis intervention and management for patients in acute distress.
• Collaborate with medical staff to ensure effective case management and continuity of care.
• Maintain accurate medical documentation in compliance with HIPAA regulations.
• Engage in clinical research to enhance treatment methods and patient outcomes.
• Offer support for families dealing with Alzheimer’s care, dementia care, and other age-related challenges.
• Utilize EMR systems for data collection and management of medical records.
• Participate in group therapy sessions and provide motivational interviewing techniques to encourage patient engagement.
• Assist patients transitioning from inpatient to outpatient settings or post-acute care facilities.

Job Types: Per diem, Contract, PRN

Pay: Up to $80.00 per hour
