ACE Home Health Care & Hospice is seeking a per diem Medical Social Worker (MSW) for home health care in Santa Clara County: San Jose, Morgan Hill and Gilroy areas

Requirements: The ideal candidate must be a graduate of an accredited school of social work. At least 2 years' experience working in social work, in a home health or acute care setting. Must have effective communication skills, knowledge of current community services available, be reliable and have a positive attitude, which fosters a cooperative and positive atmosphere.

Knowledge: Must have the clinical knowledge and critical thinking ability to effectively plan and provide coordination of patient care consistent with standards and regulations; must have exceptional interpersonal and customer services skills; must be able to effectively solve unique problems as they arise or identify when to consult the intake supervisor. Must have knowledge of current Medicare, Medi-Cal, State and Federal regulations.

Compensation:

  • Estimated Annual Full Time Compensation: $140K to $204K (This position is paid on a per-visit basis plus mileage)
  • Mileage reimbursement at federal rate
