Job Summary:

The Medical Social Worker (MSW) in a home health setting provides comprehensive social services to patients and families, focusing on addressing emotional, psychological, and social issues that may affect their health and well-being. This position works closely with the multidisciplinary team to ensure that patients receive the necessary resources, education, and support while transitioning through care at home.

Key Responsibilities:

Patient Assessment:

  • Conduct social assessments for patients admitted to home health services, identifying social, emotional, and environmental factors that affect the patient’s well-being and care plan.

Care Planning and Coordination:

  • Collaborate with physicians, nurses, and other healthcare providers to develop and implement patient-centered care plans. Assist in determining the social, emotional, and financial needs of patients.

Support Services:

  • Provide counseling and emotional support to patients and families, helping them cope with illness, disability, or death. Offer crisis intervention when necessary.

Resource Referral and Advocacy:

  • Identify and assist patients and families in accessing community resources, including financial assistance programs, home healthcare services, transportation, and housing.
  • Advocate for patients to ensure they receive appropriate services and support.
  • Discharge Planning:
  • Assist with planning for discharge by identifying ongoing home care needs and ensuring adequate support systems are in place for the patient’s transition to home care or a lower level of care.

Documentation:

  • Maintain accurate and timely documentation in accordance with federal, state, and agency guidelines, including progress notes, assessments, and care plans.

Education:

  • Provide education to patients and families about available community resources, coping strategies, and the patient’s condition, helping them understand the impact on their health and lifestyle.

Collaboration:

  • Work closely with other members of the home health team to provide comprehensive care, ensuring continuity of services and addressing any issues that arise during patient care.

Qualifications:

Education:

  • Master’s Degree in Social Work (MSW) from an accredited program.

Licensure/Certification:

  • Current state licensure as a Social Worker (LCSW, LISW, or equivalent).

Experience:

  • At least one year of experience in a healthcare setting, preferably in home health, hospice, or hospital discharge planning.

Skills:

  • Strong interpersonal skills and ability to work effectively in a team. Ability to assess and address a variety of psychosocial issues. Knowledge of social work theory and practice as it applies to home healthcare.

Other:

  • Ability to work independently, manage a case load, and handle sensitive and confidential information.

Physical Requirements:

  • Ability to travel to patient homes.
  • May involve some lifting, bending, and walking.

Education:

  • Master's (Required)

Experience:

  • Healthcare Social Work: 1 year (Required)

Language:

  • English (Required)
  • Spanish (Preferred)

License/Certification:

  • Licensed Certified Social Worker (Required)
