Position Overview

The Client Care Manager serves as the primary point of contact for clients and their families. This role is responsible for coordinating care services, managing client relationships, overseeing care plans, and ensuring client satisfaction and compliance with company standards.

Key Responsibilities

Serve as the main liaison between clients, families, caregivers, and healthcare professionals
Conduct initial client assessments and create personalized care plans
Monitor ongoing care and adjust plans as needed
Coordinate caregiver scheduling and ensure proper client-caregiver matches
Perform quality assurance visits and maintain regular client contact
Address and resolve client concerns or issues promptly
Ensure compliance with all home health care regulations and company policies
Maintain accurate records and documentation

Qualifications

2+ years of experience in a healthcare, home care, or client service management role
Strong interpersonal and communication skills
Knowledge of home health care industry regulations and standards (preferred)
Proficient in scheduling software and electronic health records (EHR)
Excellent organizational and problem-solving abilities
Compassionate, patient-centered mindset
Bachelor's degree in healthcare administration, nursing, social work, or related field (preferred)
