Job Overview
We are seeking an ECM Lead Care Manager with proven experience in working with managed care plans, claims, outreach, and case management. The ideal candidate is detail-oriented, compassionate, and skilled in navigating the healthcare system to connect clients with the services they need.

Duties

• Coordinate and oversee care management for ECM participants.
• Complete RTF and submit successfully
• Attend all trainings for managed care plan
• Communicate with managed care plans to ensure smooth coordination of benefits and services.
• Process and follow up on claims and related documentation.
• Conduct outreach to engage and enroll eligible individuals in the ECM program.
• Perform comprehensive assessments and develop individualized care plans.
• Track progress, update care plans, and document all activities in compliance with program guidelines.
• Collaborate with healthcare providers, community resources, and the internal care team.
• Communicate with proper personnel to propose any updates or changes.

Qualifications

• Required: Experience with managed care plans, claims, outreach, and case management.
• Bachelor’s degree in Social Work, Public Health, or related field required.
• Strong communication and organizational skills.
• Ability to work both independently and collaboratively.
• Knowledge of Medi-Cal and ECM program guidelines preferred.
• Bilingual skills are a plus.

Job Type: Full-time

Pay: $23.00 - $25.00 per hour
