LRH is seeking a Full Time, Licensed Clinical Social Worker to work as part of our Collaborative Care Model for behavioral health. This position will be based in the primary care office setting and will work closely with the primary care providers and Advanced Practice Psychiatric Nurse Practitioner. Psychiatric collaboration and oversight will be provided through a relationship with Dartmouth Health. This model of care encompasses a structured approach to mental health care delivery that promotes systematic communication among team members during and outside of face-to-face patient encounters.
Required Education/Course(s)/Training:
- Masters Prepared Licensed Clinical Social Worker
- Current NH License
Experience/Specialized Skills:
- Ability to perform consultations and give provider feedback on demand.
- Excellent working knowledge of behavioral medicine and evidence based treatments for mental health conditions.
- Provides timely and effective counseling to patients.
- Working knowledge of psychopharmacology required.
- Medical Social Work and Case Management experience preferred.
- Demonstrated ability to de-escalate tense situations and resolve conflict in a timely manner.
- Excellent verbal and written communication skills.
- Works with collaborative/team approach and is able to facilitate team or group activities.
- Develops and maintains knowledge about available community resources.
