What You Will Do as a Hospice Social Worker

As a Hospice Social Worker, you will be responsible for assessing the psychological status as it relates to the patient’s illness and environment, carrying out social evaluations and providing information and services to the patient and families. Some of your primary responsibilities will include:
- Assessing the psychological status of patients related to the patient’s illness and environment and communicating findings to the registered nurse.
- Carrying out social evaluations and planning interventions based on evaluation findings.
- Maintaining clinical records on all patients referred to social work.
- Serving as a liaison between patients or families and community agencies.
- Participating in the development of the total plan of care and case conferences as required.
- Assisting physician and other team members in understanding significant social and emotional factors related to health problems.

Education, Licensure & Certification Requirements
- Master’s degree in social work accredited by the Council on Social Work Education or a baccalaureate degree in psychology, sociology, or another related field.
- At least 1 year of social work experience in a healthcare setting.
