Summary
The RN Care Manager assesses participant needs, delivers nursing and healthcare interventions, and evaluates outcomes on an ongoing basis. In collaboration with an interdisciplinary team (IDT), you’ll develop and implement individualized care plans, delegate tasks to aides based on care plans, and coordinate care across the clinic/center, contracted facilities, and participant homes.
Key Responsibilities
- Provide high-quality clinical care as a member of the IDT in clinic/center, contracted facilities, and participant homes per plan of care.
- Participate in a 24/7 on-call triage rotation.
- Assess, plan, and coordinate home care services; recommend interventions to the IDT.
- Monitor acute/chronic needs in all settings to promote safe community-based care or optimal facility-based care when indicated.
- Ensure timely provider follow-up on specialist visits; assist with obtaining reports, facility documents, and labs.
- Reconcile facility MARs monthly for assigned panel; escalate discrepancies.
- Notify participants of normal test results.
- Complete timely, accurate nursing assessments; implement nursing care-plan interventions.
- Educate participants/caregivers on self-care, medications, infection control, safety, and healthy lifestyles.
- Review and revise goals and approaches with the participant, family/caregiver, and IDT.
- Evaluate CNA/Home Care Aide competence and delegate appropriately.
- Serve as Home Care Coordinator or other IDT functions as licensed/competent when needed.
- Participate in IDT, utilization review, quality improvement, trainings, and orientations.
- Follow infection control, safety, and confidentiality policies; practice standard precautions.
- Perform other duties as assigned.
Qualifications
Required
- Active State RN license.
- BLS (or obtain within 90 days of hire).
- 1+ year of RN experience.
- Valid driver’s license, reliable transportation, and state-required auto insurance.
- Ability to use EMR and standard office applications (Outlook, Word, Excel, PowerPoint).
Preferred
- BSN.
- 1+ year working with frail/elderly populations (PACE, home care, long-term care, or managed care).
- Experience coordinating care for chronic conditions (e.g., CHF, COPD, diabetes, hypertension, renal disease, neurologic disorders).
Skills & Competencies
- Clear, respectful, people-first communication; collaborative IDT mindset and shared decision-making.
- Knowledge of evidence-based geriatric care, person-centered planning, and community-based services.
- Comfort in managed-care environments (quality indicators, cost-effective care, capitated models).
- Adaptable, innovative problem-solver with sound clinical judgment and follow-through.
- Proficiency with EMR and documentation; strong organization and time management.
Work Environment & Physical Demands
- Clinic/Adult Day Health Center setting with moderate noise and controlled temperatures; occasional travel to homes/facilities.
- Lift/carry up to 30 lbs using proper body mechanics; frequent standing, walking, and use of clinical equipment.
- Potential exposure to bodily fluids/communicable diseases; standard PPE and safety protocols followed.
- Role may involve tight deadlines and dynamic environments.
Pre-Employment Requirements
- Medical clearance for communicable diseases and required immunizations prior to direct participant contact.
- Background screening, which may include Medicare/Medicaid exclusion checks, license/education verification, references, and drug screen.
