This position integrates medical and social services, facilitates communication among health providers, assesses patient needs, and connects families to various resources including medical, social, emotional, and financial support. The Coordinator follows up to ensure understanding and linkage with recommended resources. Essential skills include knowledge of family resources, experience in health care coordination, strong communication and interpersonal skills, and the ability to work collaboratively. Bachelor's degree in social work

Minimum 2 years or more of experience in health care coordination, particularly with children.
