About Job

The Care Coordinator role focuses on supporting children and families needing behavioral health care by ensuring they receive necessary health services and bridging the gap between behavioral health and primary care providers. This position integrates medical and social services, facilitates communication among health providers, assesses patient needs, and connects families to various resources including medical, social, emotional, and financial support. The Coordinator follows up to ensure understanding and linkage with recommended resources. Essential skills include knowledge of family resources, experience in health care coordination, strong communication and interpersonal skills, and the ability to work collaboratively. The role aligns with the mission, vision, and values of Tampa General Hospital.

Bachelor's degree in social work

Minimum 2 years or more of experience in health care coordination, particularly with children.

Technical Knowledge, Skills, And Abilities

  • Technical Skills: Ability to organize and coordinate care activities among various health care providers and services. Capability to follow up with primary care providers, clients, and families to ensure understanding and implementation of care recommendations. Strong problem-solving skills to address and overcome barriers to care. Experience with enterprise-level applications and systems, such as Electronic Health Records (EHR).
