- Meeting with each new patient to assess for needs and provide resources, as appropriate
- Liaising with or working as a financial advocate to provide information and support for medical and insurance coverage and securing drugs(e.g., accessing co-pay assistance, prior authorization)
- Providing psycho-social-emotional-spiritual support and skill-building for individuals and families coping with a cancer diagnosis throughout the disease continuum
- Assisting with decision-making and life-planning to include advance directives. Helping patients transition to palliative care, hospice, or survivorship.
- Assist uninsured patients with application to Medicaid and other funding sources for care
- Assess need for transportation and arrange all transportation for patients for all appointments
- Collaborate with Director of Patient Services to identify the need for patient support programs and refer patients to ACC programs
- Serving as a patient and family advocate within the healthcare system and coordinating care
- Performing distress screening assessments to identify and address stressors
- Ensure clinical productivity is maintained to be at least 6.5- 7 hours of scheduled “billable” hours per workday or 33- 35 hours per week. Develop a system to measure and monitor productivity. Fill clinic time with alternative payor mix if time permits or caseloads change due to non-utilization by patients to ensure high productivity/service to the clinic. Develop/ report monthly data reflecting productivity to supervisor and assigned medical directors.
