TDT Case Manager (QMHP-C)

Succor Behavioral Services is currently seeking Qualified Mental Health Professionals that have a passion to serve youth and provide quality care. This position is for school based therapeutic day treatment.

Qualifications:

  • Bachelor’s Degree in Social Work, Special Education, Psychology, or other Human Services related field, and minimum of one-year experience providing day treatment services to children and/or adolescents and/or meet the criteria of a Qualified Mental Health Professional (QMHP) per DMAS licensure standards under the direct supervision of a QMHP.
  • Physical Restraint – Handle With Care, CPR and First Aid Training
  • Medication Management Certification, if applicable
  • Must hold a valid, current Commonwealth of VA Driver’s License and be insurable to drive a company vehicle
  • Bloodborne Pathogens, Infection Control, Emergency Preparedness, Human Rights, HIPAA Training and Decision Driving
  • Other requirements as assigned by DMAS and/or licensure standards

Duties and Responsibilities:

  • Responsible for day-to-day operations for assigned group of clients
  • Implement Behavior Modification System
  • Complete all required documentation which includes but not limited to, the tracking of all individualized behavior programs, daily point sheets/totals, outcome tracks, discharge planning, progress notes, treatment plans, quarterlies and all related documentation in compliance with DMAS, Licensure and Succor’s standards
  • Maintain weekly contact with clients’ primary caregivers, school personnel and other relevant health care professionals. Schedule and attend meeting as required
  • Attend school meetings (i.e. IEP, child studies, etc.) upon approval of Clinical staff or Program Coordinator
  • Attend monthly meetings with Clinical staff and Program Coordinator for supervision and chart audits.
  • Establish working relationships with representatives of other agencies who work with and provide services to individuals.
  • Assist in the referral process for each client
  • Provide transportation for clients using company vehicles as needed
  • Perform other duties as assigned
