Social Services – LMSW / RN Case Manager

Shift: Day – 5x8-Hour (08:00 AM – 4:30 PM)

Job Description:

We are seeking a Licensed Masters of Social Worker (LMSW) or RN Case Manager to join the team at MedStar Washington Hospital Center. In this role, you will be responsible for acute hospital discharge planning, coordinating care, obtaining facility choices, sending referrals, and managing Medicare letters (IMM and others).

Responsibilities:

• Conduct patient assessments and coordinate discharge planning for adults and geriatric populations
• Collaborate with interdisciplinary teams to ensure smooth patient transitions
• Refer patients to rehab and long-term care facilities
• Maintain accurate documentation in Cerner (preferred)
• Participate in weekend rotation (one shift every 5–6 weekends)

Requirements:

• Active State License (LMSW, MSW, LCSW, or LCSW-C accepted) or RN license
• BLS certification
• Minimum 2 years of acute hospital discharge planning experience
• Travel experience required
• Cerner experience preferred
• Strong skills in care coordination and patient advocacy
