Job Requirement
Position Summary:
Performs professional level casework in specialized areas such as: Cystic Fibrosis Clinic, Pediatric Oncology Clinic, Rehabilitation, HIV Clinic and Kidney Transplant Program assessing patient needs and coordinating a safe, comprehensive discharge plan which may involve emotional support to the patient, the family or significant others.

- Recent documented experience in an acute care setting (hospital, or similar setting).
- Experience with pre and post op liver patients
Knowledge, Skills, Abilities, and Physical Requirements
Education/Experience:
Equivalent to a Master’s Degree in Social Work and two (2) years of professional level social work experience in a social service agency or hospital setting.
Knowledge of:
Principles, practices and techniques of social work, health and welfare; federal, state and county regulations governing area of responsibility; local, state and Federal social welfare and assistance programs; community resources; medical terminology, diagnoses, and treatments; counseling techniques such as family dynamics, cultural diversity and crisis intervention; assessment and interviewing techniques; department and hospital safety practices and principles; patient rights; infection control policies and practices; handling, storage, use and disposal of hazardous materials; department and hospital emergency response policies and procedures.
Skill in:
Developing and implementing individual treatment plans; coordinating an intervention; solving problems; applying effective counseling techniques; communicating effectively with a wide variety of people from diverse socio-economic and ethnic backgrounds; establishing and maintaining effective working relationships with all personnel contacted in the course of duties; preparing written records and reports; efficient, effective and safe use of equipment.
Physical Requirements and Working Conditions:
Mobility to work in a typical office setting, and use standard office equipment; vision to read printed materials and a VDT screen; hearing and speech to communicate in person and over the telephone. Exert up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a eligible amount of force constantly to move objects.
