Job Summary: Discharge planner/Social Services Assistant
The Social Services Assistant supports the Social Services and Billing Department in maintaining regulatory compliance in resident rights, consents, resident satisfaction and CMS required documentation. The Director of Social Services oversees the Social Services Assistance performance of their job duties: examples including but not limited to: timely delivery of CMS required notices, conducting of surveys and consents for mental health services
Essential Job Duties & Responsibilities:
- Meet with residents and families to explain rights as a Medicare recipient
- Explain and issue Notices of Medicare Non-Coverage (NOMNC), Advance Beneficiary Notices (ABNs), Insurance Rate Sheets, and Level of Care Determinations (LOCD).
- Support families in coordination of Medicaid process
- Scheduling of Admission Care Conferences
- Support Social Services in clerical functions of faxing referrals,
Required Skills/Abilities:
- Excellent verbal and written communication skills.
- Excellent organizational skills and attention to detail.
- Excellent time management skills with a proven ability to meet deadlines.
- Ability to prioritize tasks.
- Ability to function well in a high-paced and at times stressful environment.
- Proficient with Microsoft Office Suite or related software.
Supervisory Responsibilities:
- None.
Travel Requirements:
- None
Education and Experience:
- High school diploma
- Preferred Associates Degree in Human Services Related field
- Ideal Social Services Technician Registration with State of Michigan
Physical Requirements:
- Spend approximately 6 to 8 hours on floor.
- Must be able to lift up to 15 pounds at times.
