About the Role
We are seeking a compassionate and experienced Lead Care Manager to join our team and provide family-centered, member-focused, relationship-based care coordination services to adults, children, and youth enrolled in Enhanced Care Management (ECM). This role is essential in ensuring seamless, high-quality care that meets the unique needs of our members.
Key Responsibilities
- Coordinate care across service providers and support networks to ensure continuity and avoid duplication.
- Actively engage ECM-eligible members and oversee the implementation of individualized care plans.
- Deliver services in locations most accessible to members, including homes, clinics, and community settings.
- Connect members to social services, transportation, and other supports.
- Advocate for members with healthcare professionals and institutions.
- Apply motivational interviewing, trauma-informed care, and harm reduction strategies.
- Collaborate with hospital staff on discharge planning and accompany members to appointments as needed.
- Monitor treatment adherence and provide health promotion and self-management education.
- Manage a caseload of 52–60 ECM members, adjusting as directed by senior management.
- Identify service gaps and barriers, updating care plans and coordinating with multidisciplinary teams.
- Participate in supervision meetings and contribute to team discussions and morale.
- Uphold ethical standards and agency culture in all interactions.
- Ensure compliance with HIPAA, confidentiality, and other legal mandates.
- Support agency performance quality improvement (PQI) goals.
- Maintain punctuality and reliable attendance.
Qualifications
- Proven experience in care coordination, case management, or a related field.
- Excellent communication, organizational, and advocacy skills.
- Ability to work independently and collaboratively in a fast-paced environment.
- Sensitivity to cultural and socioeconomic diversity.
- Familiarity with motivational interviewing, trauma-informed care, and harm reduction approaches.
- Bilingual proficiency (English/Spanish) is a plus.
- Valid driver’s license and reliable transportation for field visits.
