For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness.
Through the Benson Ridge and the Southwest Queens Senior Case Management Programs, case management and the homecare services are offered to the homebound elderly. The services include offering care management plans, assessments for the homecare, connections with the Home Delivered Meals and Friendly Visiting. The goal of the Case Management Program is to provide vital support services to keep frail seniors living safely in their homes and communities.
STATEMENT OF THE JOB
The Intake Coordinator provides a comprehensive review of Case Management services and screens potential older adult callers for eligibility for services. If deemed eligible, the intake worker inputs critical information to initiate the intake process and case enrollment in the NYC Aging database after obtaining the consent from the caller. The Intake Coordinator must also be able to communicate the case management services to other callers from the community.

  • Perform intake activities, which include assessing eligibility, preliminary evaluation of the presenting problem, initiating services as needed and providing information and referral services.
  • Maintain a current resource book of available providers and community resources for callers
  • Providers callers with information on entitlements and community-based programs and resources
  • Provide emergency coverage for Home Delivered Meal clients when the client fails to answer the door, when needed.
  • Maintain up to date: case notes, waiting lists for clients and the timely submission of monthly statistics and reports as required by the agency and the funding source.
  • Attend trainings including yearly training hours required by NYC Aging and other agency trainings/meetings.
  • Utilize the Vive data system for client information and screen for benefits with UNIFORM system.

QUALIFICATIONS

  • Bachelor’s Degree in Human Services or related services from an accredited college or university.
  • Knowledge of entitlements for older adults preferred.
  • Good verbal and written command of the English Language.
  • Proficient in computer software applications, Excel, and Microsoft Office.
  • Within 6 months of hire date learn the NYC Aging client software system for data entry of client files
  • Bilingual preferred.
  • Ability to lift up to 10lbs.
  • Must be able to travel to conferences and other locations for trainings and meetings as needed
