We are currently seeking a dedicated and organized Intake Coordinator to join our team and serve as a vital connection between clients, caregivers, and agency operations.

Position Summary

The Intake Coordinator plays a key role in initiating services for new clients, ensuring all necessary documentation is completed, and coordinating with various departments to guarantee smooth service delivery. This role involves regular communication with clients and home visits to assess care needs and ensure readiness for services.

Key Responsibilities

  • Conduct intake calls and gather essential client information.
  • Perform home visits to assess client needs, complete intake documentation, and evaluate home environments.
  • Coordinate and schedule assessments and services.
  • Ensure accuracy and completeness of all intake forms and records.
  • Communicate with clients, caregivers, and case managers in both English and Spanish.
  • Maintain client records in compliance with agency policies and regulatory requirements.
  • Follow up on referrals and assist with onboarding new clients.
  • Collaborate with team members to ensure client satisfaction and timely service delivery.

Requirements

  • Bilingual in English and Spanish (required)
  • Must be 21 years of age or older
  • Valid Driver’s License
  • Strong computer skills, including proficiency in Microsoft Office (Word, Excel, Outlook, Teams)
  • Experience using HHAeXchange software (preferred)
  • Excellent interpersonal, communication, and organizational skills
  • Ability and willingness to conduct home visits in the community
  • Preferred: Previous experience working in a homecare agency office, particularly in intake, coordination, or administrative support roles

Job Type: Full-time

Pay: From $14.00 per hour

Expected hours: 40 per week
