Description

We are currently seeking a talented, compassionate Clinical Social Worker to provide care to patients & families in the counties that we serve. This position will be part of the Chesapeake Supportive Care (Palliative) and Medical Services Team.

Want to join our Team? We are currently looking for dedicated Clinical Social Workers to join our team. As a Clinical Social Worker, you will provide psycho-social supports and interventions for those with life-limiting illnesses, helping patients and their caregivers navigate end-of-life care with compassion. You will work with your team's RN Case Managers, Chaplains, and CNAs, as well as our Hospice Physicians, to provide physical, psychosocial, and spiritual aspects of care. We empower our Social Workers to take ownership of their day and schedule with an emphasis on care continuity and quality.

Office Location: Largo, MD

Coverage Area: Anne Arundel, Prince George's, Charles and Calvert Counties

Status: Full-time, Monday - Friday 40-hour work week

Main Responsibilities:
  • Provides psychosocial services to patients and their families in the assigned location.
  • Conducts assessments and follow up visits in collaboration with the supportive care team and population health teams.
  • Facilitates completion of Advanced Directives and MOLST forms as needed.
  • Formulates individualized plans of care for patients based on assessed needs, strengths, limitations and goals. Includes patient and caregivers in the care planning process and and facilitates discussions regarding goals of care. Ensures that patient's desires are taken into account when the treatment plan is formulated. Regularly reassesses for changing needs.
  • Participates actively in IDT meetings, functioning as an advocate for the patient and family. Communicates initial/ongoing plan of care appropriately to multidisciplinary treatment team.

  • Provides counseling and emotional support and/or crisis intervention as per professional standards.
  • Engages with patient/caregivers to develop a rapport and a sense of trust in the care being given.

Qualifications

Your Qualifications
  • Masterof Social Work degree from an accredited educational institution required. LCSW-C required. ACHP-SW preferred.
  • Minimum 2 years of experience in the health care field.Prior hospiceor Supportive Care experience preferred.
  • Demonstrated ability to provide quality patient care. Possess ability to assess patient's needs, e.g. physical, teaching, mental and spiritual.
  • Must have strongclinical skills and creative problem-solving skills, with the ability to prioritize and delegate..
  • Valid driver's license and acceptable driving record and reliable transportation
  • Ability to meet the physical demands of the position with or without reasonable accommodation.
