As well as assessment of client’s life domain areas; linkage to services in the community; income assistance and employment; assistance coordination of service linkage; medical and medication needs in coordination with client medical needs; home visits/ inspections; gather, enter, and document client information and interactions; and participation in local Continuum of Care meetings in order to enhance communication and the continuity of care.
Case manager will also provide or arrange transportation for clients to and from treatment, medical appointments, and other community resource appointments, and monitor client compliance with program expectations.

Required Educational/Experiential Qualifications
- Graduation from a four-year college or university in human services or related field
- They must have experience and skills in substance use and/or mental health case management and be trained to provide evidence-based care coordination, clinical screens, and to support treatment recommendation follow through. The Housing Case Manager will possess the interpersonal skills required to comfortably approach and engage with the client population in diverse settings.
- An equivalent combination of education and experience.
- Coordinates admissions to the HHH Program.
- Participate in admissions and discharges for the program and when necessary, responds to emergency client needs such as medication and food.
- Assists clients in accessing pharmaceutical care charity programs, or charity care when applicable. Works with medical staff to coordinate services for the client.
- Develop and maintain a network of supportive agencies in order to ensure necessary referrals for basic needs and other services that could include employment or school.
- Communicate and collaborate with apartment owners and landlords, to secure safe housing, to address any conflicts or problems, and to maintain positive relationships with housing agencies.
- Engage in regular consultation with team members and supervisor to ensure ethical and high-quality services and documentation.
- Coordinate with QRT, ORHP and Homeless Outreach Teams at Coastal Horizons and attend weekly team meetings
- Conducts monthly case management sessions with participants to further goals of permanent supportive housing.
- Transport clients utilizing a company car or van to and from appointments.
- Is subject to both inside and outside environmental conditions and exposed to hazards such as fumes, odors, dusts, mists, gases or poor ventilation and may require the employee to work in close quarters.
- May be exposed to human body fluids and is subject to the OSHA requirements on blood borne pathogens.
- Ability to assertively interact with and monitor client behavior.
- Strong interpersonal communication skills; ability to develop and maintain professional boundaries and to interact effectively with clients and staff.
- Knowledge of available community resources and services to support clients with substance abuse problems.
- Ability to represent the Agency in community forums and effectively conduct advocacy efforts aimed at furthering the vision and mission of Coastal Horizons Center.
- Knowledge of the laws, regulations and policies governing mental health and substance abuse treatment, residential services and supportive housing.
- Ability to demonstrate effective oral and written communication.
- Strong organization skills; ability to develop, organize and maintain schedules.
- Knowledge and comfort in working with client population.
- Able to work with basic computer skills.
- Ability to network and coordinate efforts between treatment, medical, and other support systems for the clients.
- Must be able to physically perform the basic life operational functions of climbing, stooping, crouching, reaching, pushing, pulling, lifting, standing, walking, fingering, grasping, talking, and hearing.
- Must be able to perform sedentary work exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift objects.
- Must possess the visual acuity to perform extensive reading and determine the accuracy of work assigned.
- Must have a valid NCDL and a good driving record approved by CHC insurance provider.
- Effective placement and retention of clients in the housing program
- Completion and acceptance of the Annual Performance Report by HUD yearly
- Meeting or exceeding program goals with clients/ participants in the program
- Community liaison and involvement with committees that further the goals of the clients in the community and decrease return to higher levels of care and/ or homelessness
- Decreased number of clients returning to higher levels of care by improving service delivery and needed one to one attention with higher needs clients
- Increased connections in the community that support clients after discharge from the program including employment assistance or assistance with disability determination if appropriate
