Registered Nurse (RN) – Care Manager

Pay:

• Travelers (40 hrs/week): Weekly Gross $2,800 ($800 taxable + $2,000 non-taxable stipends)
• Locals (40 hrs/week): $63/hour | Weekly Gross $2,520

About the Role

Providence PACMED – Beacon Hill is seeking experienced RN Care Managers to join our Population Health team. This hybrid position offers a balance of on-site patient interaction and remote care coordination, with the mission of improving patient outcomes, reducing emergency department visits, and preventing readmissions.

You’ll work closely with patients, families, and interdisciplinary teams to ensure smooth care transitions, conduct medication reconciliation, and coordinate post-discharge follow-up—especially for those with chronic conditions.

Schedule

• Monday–Friday, 8:00 AM – 4:00 PM
• Hybrid: 2 days/week on-site at Beacon Hill clinic, 3 days/week remote
• No weekends, no on-call requirements

Key Responsibilities

• Coordinate care transitions for all inpatient discharges (except labor & delivery and oncology follow-up)
• Manage post-ED visits with focus on chronic condition patients
• Conduct admission criteria reviews, care planning, and discharge planning
• Perform medication reconciliation
• Apply InterQual and Milliman guidelines
• Ensure compliance with CMS, HIPAA, Joint Commission, OSHA, and other regulatory requirements
• Collaborate with social services and other disciplines for comprehensive patient care

Requirements

• License: Active RN license (WA or Compact)
• Experience: Minimum 1 year in acute care, LTAC, rehab, or skilled nursing
• Certifications: BLS required
• Systems: Epic EMR experience required
• Reliable personal transportation (not public transit)

Preferred Skills

• Case management / utilization review experience
• Knowledge of CPT, ICD-10, DRG coding, and HEDIS measures
• Familiarity with NCQA, CMS, and The Joint Commission standards
