Responsibilities include:

Chronic care management - communicate monthly through pre-arranged telephone calls with patients enrolled in the chronic care management program.

• Phone calls include documenting the current status of the patient (medications, health questionnaires) and encouraging patients to self-manage their chronic conditions and make progress towards the goals set by the patient and their physician.

Obtaining and documenting vital signs including but not limited to: blood pressure, temperature, pulse, respiration, height, weight, health history, current medications and presenting problems.

Schedule patients for diagnostic testing and referrals.

Communicate with patients about test results and plan of care, including prescription renewals and inquiries.

Perform other related duties as assigned.

Must be able to think and work independently.

Experience with EMR preferred.
