- JOB RELATIONSHIPS
- Responsible to the Director of Care Coordination and Community Health Programs. Consults and works collaboratively with members of the Emergency Department team, Patient Care Services inpatient teams, case management team and Hospital Leadership staff as appropriate.

- DEFINITION OF POSITION
- The Behavioral Health Case Manager/Social Worker is a Masters or 4-year undergraduate prepared individual who is responsible for facilitating behavioral health care coordination services to patients and their families who present to Carteret Health Care with mental health or substance use issues.

- QUALIFICATIONS
- Professional
- Master of Social Work degree from a program accredited by the Council on Social Work Education preferred, other master’s degree in human services field or 4-year undergraduate degree in social work, psychology or other human services degree accepted. Preferred licensure as a Licensed Clinical Mental Health Counselor, Licensed Clinical Social Worker or Licensed Clinical Addiction Specialist through the appropriate professional North Carolina licensing board. Associate or Bachelor’s level clinicians must practice in accordance with requirements of the appropriate professional North Carolina licensing board and be under the supervision of an approved, fully licensed clinician.
- Two to three years of post-masters/post-bachelors experience in a health care setting such as a hospital, mental health setting, extended care facility, or home care agency.
- Knowledge of applicable legal responsibilities/restrictions and current trends in behavioral health care coordination/social work pertinent to the hospital practice setting.
- Knowledge of acute, chronic and terminal illnesses, mental and physical disabilities, and the normal developmental process and the impact on family systems.
- Evidence of participation in ongoing continuing education programs.
- Knowledge of and adherence to the National Association of Social Workers Code of Ethics or other pertinent professional Code of Ethics.
