Social Worker – Prevention Program

Location: Brooklyn, NY
Compensation: $58,000–$65,000/year

Hours: 35 hours, exempt

Position Summary

As a Social Worker, you will carry a caseload of 8 families, delivering home-based counseling and support services rooted in the Solution-Based Casework (SBC) model. You will work closely with families facing challenges related to substance use and/or mental health, helping them build safety, stability, and long-term well-being.

Key Responsibilities

  • Provide in-home individual, family, and group counseling using SBC principles.
  • Conduct screenings for mental health, substance use, and child safety.
  • Support families through crisis intervention, advocacy, and resource referrals.
  • Collaborate with ACS, treatment providers, schools, and legal systems.
  • Maintain accurate documentation in PROMIS, Connections, and internal systems.
  • Participate in case consultations, SBC training, and team meetings.

Qualifications

  • Master’s Degree in Social Work, Psychology, Mental Health Counseling, or related field (required)
  • LMSW eligibility strongly encouraged (exam sponsorship available; terms apply)
  • Child welfare experience preferred
  • Bilingual in English/Spanish preferred
  • Must be available at least two evenings per week
  • Strong organizational, communication, and counseling skills
  • Commitment to trauma-informed, strength-based practice
  • SBC training or certification a plus
