Position Overview:
IU Health’s Congregational Care Network (CCN) partners with local congregations to support older adults with chronic illnesses who experience social isolation or loneliness. The Social Worker will play a pivotal role in connecting patients to community resources, collaborating with faith-based partners, and supporting interdisciplinary teams to ensure safe, timely, and personalized care. This role involves assessing psychosocial needs, counseling, and mobilizing community support to improve quality of life.

Position Overview:

Key Responsibilities:
- Collaborate with faith-based partners, community agencies, and healthcare teams to deliver personalized support.
- Assess patients’ and families’ psychosocial risks, prior functioning, and support systems.
- Counsel patients and families on emotional, social, and financial impacts of illness or disability.
- Access and mobilize community resources to meet identified needs.
- Communicate complex patient and family needs effectively and model caring practices.
- Provide short-term therapeutic counseling and crisis intervention as needed.
- Maintain strong relationships with community organizations, law enforcement, APS/CPS, schools, and probation agencies.
- Ensure seamless coordination of care and resources for socially isolated older adults.
- Education:
- Master’s Degree in Social Work (MSW) required.
- Licensure:
- Permanent Licensed Social Worker (LSW) required.
- LCSW licensure preferred.
- Experience & Skills:
- 0-3 years of relevant healthcare, mental health, or community-based social work experience.
- Knowledge of disease impact, human behavior, and legal mandates related to protective services.
- Knowledge of growth and development principles over the lifespan.
- Proficiency in crisis intervention and de-escalation techniques.
- Proficiency in Microsoft Excel and Word.
- Relevant experience working with community resources and faith-based organizations.
- Certifications:
- De-escalation certification preferred.
