Job Description: The Mental Health Counselor provides support to enhance the quality of patient care by working directly with the patients in a treatment setting and collaboratively as a member of the program's multi-disciplinary team in accordance with the Discover Health Group Care Model. He/she is responsible for collaborating with the Clinical Team with the direct clinical patient care by providing support and guidance to patients to promote engagement and continuity of care. Working in conjunction with the multidisciplinary team, she/he will help provide a safe and therapeutic treatment experience for the patients served.
Responsibilities:
- Ensure clinical services are in compliance with state and federal regulatory agencies.
- Ensure adherence to the required HIPAA and documentation requirements as identified by state and federal regulatory agencies.
- Appropriately and accurately document all patient interactions into EMR for continuity of care.
- Perform initial consultations and mental health assessments to understand clients' needs and diagnose mental health conditions.
- Create and implement personalized treatment plans tailored to each client's unique circumstances and needs with fidelity such that the comprehensive care of clients reflects the philosophy and objectives of Discover Health, LLC.
- Provides direct support to patients by providing 1-to-1, group or family therapy sessions using various therapeutic techniques such as cognitive-behavioral therapy (CBT), psychodynamic therapy, and others.
- Facilitate individual, group, or family therapy as scheduled/assigned.
- Supports the clinical/case management teams as directed by the Clinical Director by providing direct support to patients to help encourage, engage plan, and complete continuity of care plans while collaborating with the patient's treatment team.
- Regularly document and evaluate clients' progress, adjusting treatment plans as necessary to ensure effective care.
- Provide information and support to clients and their families about mental health conditions and coping strategies.
- Engages patients by conducting recovery-oriented activities as scheduled/assigned.
- Participate in all department/team meetings.
- Duties, responsibilities and activities may change at any time with or without notice as assigned.
Minimum Qualifications:
- A master's degree in mental health counseling, psychology, social work, or related field (required.)
- Licensed Clinical Social Worker (LCSW), LCMHC, or equivalent state licensure (Preferred)
- Minimum of 1-3 years’ experience treating individuals in mental health settings.
- BLS/CPR Certification.
- Knowledge of mental health disorders, treatment methodologies, and ethical guidelines.
- Familiarity with evidence-based practices (e.g., CBT, DBT, Motivational Interviewing).
- Proficiency in using therapeutic assessment tools and techniques.
- Comfortable working with diverse populations including LGBTQ+, veterans, and justice-involved clients.
- Ability to handle sensitive information with confidentiality.
- Excellent listening, verbal, and written communication skills.
- Empathy and interpersonal skills are essential.
- Dynamic and enthusiastic personality.

Work Environment:
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets. The noise level in the work environment is usually moderate.
Physical Demands:
While performing the duties of this position, the employee is regularly required to talk or hear. The employee frequently is required to use hands to finger, handle or feel objects, tools or controls. The employee is occasionally required to stand; walk; sit; reach with hands and arms; climb or balance; and stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move objects up to 25 pounds. Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision and the ability to adjust focus.
Travel:
Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected.
