Job Summary
At Caring Hearts of Texas, our solemn duty is to provide compassionate care and support to patients and their families during the end-of-life transition. With empathy and care, our mission is to be a steady, supportive presence for patients and families in their time of need, reminding our community that there are still caring hearts to lean on. We are looking for a Intake Coordinator to join our team and help us provide caring, compassionate, end-of-life care to terminally ill patients, focusing on managing symptoms, pain relief, and offering emotional support to both the patient and their family, while ensuring comfort and dignity throughout the process. This position requires strong clerical skills, excellent communication abilities, and a commitment to providing high-quality patient service.

Responsibilities include:

• Ensures maximum third-party reimbursement through insurance verification and authorization processes.
• Gathers, collates, and reports referral statistics including key customer referral trends.
• Maintains comprehensive working knowledge of Caring Hearts of Texas contractual relationships and ensures that patients are admitted according to contract provisions.
• Participates in quality assessment performance improvement teams and activities.
• Scans and uploads medical records into the patients profile.
• Ensures compliance with all state, federal, and Joint Commission referral/intake regulatory requirements.

Qualifications

• Demonstrates good communications, negotiation, and public relations skills.

Job Type: Full-time

Pay: $18.00 - $23.00 per hour
