Social Worker MSW - State Source Program

Description:

RESPONSIBLE FOR:
Responsible for obtaining client information, baseline data, consent and completing appropriate forms. A primary liaison between Columbus Regional, the community, and network partners to ensure appropriate linkage of services and referrals. Coordinates program activities in preventing disease, promoting and protecting health and minimizing complications of health problems in target population. Knowledge of community resources, treatment modalities, and the present health care delivery system, both hospital and community based.

Qualifications:

MINIMUM EDUCATION REQUIRED:
Master's degree from an accredited social work program OR Graduate of an LPN program, Health related degree OR at least 2 years community health experience

MINIMUM EXPERIENCE REQUIRED:

Previous experience in community or acute care setting.

MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
None

ADDITIONAL QUALIFICATIONS:
Prefer a minimum of two (2) years of experience in an acute or post-acute setting. Licensed master social worker (LMSW) in state of Georgia preferred.
