Job Summary
The Community Bridges, Inc. (CBI) Client Care Coordinator is a shelter or temporary lodging site-based position responsible for managing a housing caseload. The role involves coordinating care and support services for individuals and families experiencing homelessness, especially those with complex substance use, mental health, housing, and medical needs. The coordinator ensures connection to PCMH and PCP services, working with individuals with high acuity and vulnerabilities.
The position works as part of a multidisciplinary team, including Patient Care Techs, Registered Nurses, Counselors, and Clinical Leads.
Skills/Requirements
- High school diploma or GED required; associate degree or higher preferred.
- Minimum of six months recovery from substance use and/or mental health disorders required.
- At least six months of paid related work experience in behavioral and medical crisis situations required.
- 1-3 years of healthcare-related work experience preferred.
- Valid identification required.
- Completion of CBI Peer Certification within 90 days of hire.
- Arizona Fingerprint Clearance Card (Program Specific).
