Care Advantage Skilled is looking to add a Medical Social Worker to our team. This position would be PRN, and would cover the Richmond, North Chesterfield, Chesterfield area as well as some of the surrounding areas.

Care Advantage Skilled is our Medicare-certified home health company offering a wide range of skilled care services, programs and resources aimed at restoring our patient's health and functional abilities. Job Summary

The Social Worker is under the direct supervision and direction of the Clinical Manager. The Social Worker provides psychosocial assessments/status, diagnosis and treatment to patients in their place of residence as prescribed by a physician. The Social Worker works collaboratively with all internal and external customers and partners and adheres to all Federal, State, Local and Company policies and regulations. Essential Job Responsibilities

  • Performs accurate and comprehensive assessments-assess patient's psychosocial status
  • Obtains physician orders for certification and re certification of home medical social work services-implements medical plan of care and revises as needed
  • Ensures that changes in patients' clinical status are communicated to other healthcare team members and Agency office staff verbally and in writing in a timely manner
  • Ensures that physician orders are communicated to other healthcare team members and Agency staff in a timely, accurate, and thorough manner
  • Coordinates care planning and delivery with the physician, patient and family/caregivers, other healthcare team members and Agency staff to facilitate optimal patient outcomes
  • Educates patients, their families/caregivers and other staff as appropriate, in appropriate medical social work modalities, interventions, and community resources
  • Delivers medical social work modalities and interventions appropriate to the patient's clinical status and needs, and in accordance with the standard of care for patient's
  • Provides, modifies or discontinues medical social work modalities and interventions based upon an ongoing assessment/reassessment of the patient's clinical status and in accordance with physician orders
  • Synthesizes assessment data and analyzes data to make clinical judgments regarding patient management
  • Provides medical social work modalities in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors
  • Identifies risk of injuries or threats to life and health in the patient's environment, i.e., environmental/safety/mobility risks and implements appropriate interventions to reduce those risks
  • Addresses and supports cultural practices providing such practices do not harm the patient or others, and do not interfere with the planned course of medical social work
  • Performs transfer and discharge planning functions in accordance with Agency policy and applicable laws, regulations and standards
  • Maintains currency of professional knowledge by participating in continuing education offerings
  • Ability to work with individuals to enlist cooperation of many people to perform/achieve a common goal
  • Maintains current knowledge of community resources. Identifies and accesses appropriate external and internal resources to meet patient and/or family needs and to facilitate optimal patient outcomes while home care services are being provided, and for transfer and discharge processes
  • Complies with the Agency's policies & Embrace Change
  • Adheres to all applicable laws, regulations and standards

Requirements

  • Minimum of 2 years experience in a clinical setting, case work, or counseling in a healthcare or social services delivery system
  • Master's degree in social work, sociology, or psychology from a Council of Social Work Education accredited college/university
  • Active Virginia clinical social worker license, preferred
  • Ability to apply and analyze data as it pertains to quality and compliance metrics
  • Prior home health experience, preferred
  • Acceptance of philosophy and goals of Agency
  • Ability to exercise initiative and independent judgment
