Social Worker - San Francisco
Position Summary
Provides patient assessment, counseling, and discharge planning. Acts as a liaison between the patient/family/hospital and community agencies/resources. Demonstrates expertise in social work, working as a member of an inter-disciplinary team comprised of a physician, nurses, and case management. Applies specialized knowledge of human development and behavior and social, economic, and cultural systems in directly assisting patients and families to improve or restore their capacity for social functioning. This includes counseling and use of psychosocial interventions and social psychotherapy. Occasionally plans educational offerings and provides in-services as requested.

MINIMUM QUALIFICATIONS: Bachelor’s Degree in Social Work required. Current, valid, and active license to practice as a Social Worker in the state of employment required.

Qualifications

Additional Qualifications/Skills
- Master of Social Work preferred.
- Two (2) years of experience as a Social Worker in a healthcare setting preferred.
- Ability to project a professional image. Knowledge of regulatory standards and compliance requirements. Strong organizational, prioritizing and analytical skills. Ability to make independent decisions when circumstances warrant. Working knowledge of computer and software applications used in job functions. Freedom from illegal use of and effects of use of drugs and alcohol in the workplace.
- Current, valid, and active license to practice as a Social Worker in the state of employment required.
- Regular and punctual attendance.
- English is the primary language of the facility. Ability to read and write, communicate clearly with coworkers and customers and to respond to appropriate questions regarding work process or work product.
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions and decimals as appropriate to the position.
