Description:
The Medical Social Worker is responsible for providing social services to patients in an acute care setting.
This role includes conducting initial evaluations and creating comprehensive plans of care.
Facilitation of discharge planning and coordination of services for patients upon discharge is essential.
The Medical Social Worker must document according to the plan of care, including patient and caregiver education.
Collaboration with internal and external team members is expected to ensure a seamless transition for patients.
Knowledge of cultural issues and sensitivity to patients with communication problems is required.
Experience in handling advance directives, guardianship, and power of attorney matters is important.
Participation in care conferences and coordination with rehabilitation and long-term care facilities is necessary.
This role involves managing a patient ratio of 1:26 in a short-term acute care inpatient environment.
Travel experience is preferred for this position.
