We are looking for a dedicated and compassionate Care Manager to be part of our dynamic team. This is a Full-Time role, where you'll make a significant impact by working directly with clients on their health journeys.

About the Role:

The Health Home Care Manager is responsible for providing exceptional care coordination and support to clients facing chronic medical and behavioral health challenges. You will assist clients in navigating healthcare and social service systems to enhance their overall well-being.

Key Responsibilities:

• Conduct comprehensive assessments and maintain accurate, timely case notes.
• Assist clients in enrolling in Benefit and Entitlement programs and connecting them with additional support services.
• Follow up promptly with newly referred clients to ensure they receive necessary care.
• Deliver Health Home Care Management services at various community-based locations and within designated health centers.
• Participate in case record reviews and facilitate case conferences with the multidisciplinary care team.
• Coordinate with ongoing service providers, including mental health and medical specialists, to provide comprehensive support.
• Perform field work to meet clients in their communities, fostering a supportive relationship.
• Keep meticulous records through health home EMR and prepare necessary reports.
• Collaborate with clients, their families, and providers to develop and update coordinated care plans based on client needs.
• Monitor client progress and ensure timely documentation of case notes.
• Engage in team meetings to assess client care requirements and appropriate discharge planning.

Qualifications:

A Bachelor's degree in Health or Human Services is preferred, along with two years of relevant experience. A High School Diploma or GED is required.

Work Schedule: Full-time, Monday to Friday, from 9 AM to 5 PM.

Compensation: Pay range is $23.00 - $25.00 per hour.
