- Provide home-based social services to patients dealing with end-of-life issues including care management, pain, symptoms associated with chronic and terminal illnesses, depression, anxiety, poor nutrition or a lack of resources that make managing their journey difficult
- Work as part of a hospice team which will include an RN, LPN, CNA, Spiritual Counselor, Volunteers, Hospice Aides, PT, OT or SLP, focused on the patient’s plan of care and individual goals
- Monitor a patient’s psychosocial condition and identify social and emotional needs
- Conduct patient assessments, document progress and ensure patient is moving toward goals
- Provide counseling, community resource planning, crisis intervention and advocacy
- Consult with family and caregivers on patient’s plan of care and how to help them progress
- Graduate of a Bachelor’s Degree program in social work accredited by the Council on
- Social Work Education.
- Licensed to practice as a Social Worker in the state in which employed. The license is active and in good standing.
- Hospice experience preferred
