What You’ll Do
Conduct psychosocial assessments and develop individualized care plans.
Provide supportive counseling to residents and their families.
Advocate for residents’ rights and help navigate complex care decisions.
Collaborate with the care team to ensure holistic, coordinated support.
Assist with discharge planning and help residents access community resources.
Maintain accurate documentation in compliance with state and federal guidelines.
Contribute to a respectful and inclusive environment that promotes dignity.

What You Bring
Bachelor’s or Master’s Degree in Social Work from an accredited program.
Active Social Work license in the State of Ohio (LSW or higher).
Experience in long-term care, senior services, or healthcare preferred.
Strong interpersonal, problem-solving, and organizational skills.
A resident-centered mindset and a commitment to compassionate care.
