Social Worker (Float) - PACE Program

Salary Range: $36.81 - $57.15/hour

$5,000 hiring bonus for eligible external candidates

Requirements to apply

  • Master’s Degree in Social Work
  • Active Washington licensure as one of the following:
    • Clinical Independent Social Worker (or Temporary License)
    • Clinical Independent Social Worker Associate (or Temporary License)
  • At least 2 years of experience working with geriatric populations from diverse backgrounds
This is a full-time, weekday Social Work position supporting medically complex, aging adults through the Program of All-Inclusive Care for the Elderly (PACE), a comprehensive senior care model designed to keep older adults living safely in their homes and communities. In this dynamic float role, you’ll provide clinical social work services, care coordination, and psychosocial support across multiple PACE clinic locations throughout Seattle and nearby areas, including home visits and community-based settings.

Job Overview

In this integral role on the interdisciplinary care team, the Social Worker delivers holistic support to medically complex older adults enrolled in the PACE program. Responsibilities include conducting psychosocial assessments, providing one-on-one and group counseling, coordinating community and financial resources, and advocating for participant needs across care settings, including home visits. This is an ideal opportunity for a mission-driven social worker who values long-term relationships and person-centered care.
