Seeking a full-time clinical social worker to provide services focused on substance use disorders and co-occurring behavioral health issues.

• Responsibilities include psychosocial assessments, counseling, treatment planning, crisis intervention, discharge planning, and community resource coordination.
• Utilizes SBIRT model for screening, brief interventions, and referrals, with documentation and collaboration with healthcare teams.
• Develops support groups and fosters community partnerships to enhance patient care.
• Requires a Master’s degree, LCSW licensure, and at least two years of hospital or behavioral health social work experience; experience with substance use and complex medical needs preferred.
• May involve occasional evenings and weekends.
